The Pandit Deendayal Upadhyay Rajya Karmchari Cashless Chikitsa Yojana, launched by the Uttar Pradesh Government in 2022, provides cashless and free medical treatment to serving and retired government employees and their families.

Benefits of the Scheme

Eligible serving and retired government employees in Uttar Pradesh and their families can avail:

Eligibility Criteria

To be eligible for the State Health Card, applicants must meet the following conditions:

  • Must be a UP Government employee or retiree.
  • Pension holders and their dependents are also eligible.
  • Family pension beneficiaries can also apply.

Required Documents

The following documents are necessary when applying for the State Health Card:

  • Aadhaar Card (Applicant & Dependents).
  • Mobile Number.
  • Photographs (20 KB).
  • Dependent’s Disability Certificate (if applicable).
  • Birth Certificate for children above one year of age.

Steps to Apply

Follow these steps to apply for the State Health Card:

  1. Visit the official website.
  2. Click on Apply for State Health Card.
  3. Enter your Aadhaar-linked mobile number and captcha.
  4. Verify your number via OTP.
  5. Fill out the application form and dependent details.
  6. Upload all required documents.
  7. Submit the form after reviewing the details.

Once approved, download your State Health Card from the Print State Health Card link.
